Apple's standard Licensed Application End User License Agreement applies to the app in addition to these terms.

1. Who we are

Forma 360 ("Forma", "the app") is provided by RavenIQ. Contact: support@raveniq.ai.

2. Not medical advice

Forma gives information and suggestions, not medical advice. It is not a medical device and does not diagnose, treat, cure or prevent any condition. Targets, training suggestions, recovery notes and scores (including the Healthspan score) are estimates from the numbers you enter or connect and general fitness rules, and can be wrong for you. Talk to a doctor or other qualified professional before changing your diet or exercise, especially if you are pregnant, under 18, have a medical condition or an eating disorder, take medication, or are injured. Stop if something hurts. In an emergency call your local emergency number. You use Forma at your own risk.

3. Your data

Your data is yours, stored on your phone and in your own iCloud (see the Privacy Policy). You can export or delete it in Settings. We are not responsible for data lost because you deleted the app, turned off iCloud or lost your phone without a backup.

4. Third-party data and optional AI

Food data comes from USDA FoodData Central (bundled) and Open Food Facts (looked up when you scan or search). It may contain errors; check labels. Exercise illustrations are drawn by Forma itself inside the app; see Settings > Acknowledgements.

AI is optional. If you connect an AI service of your own, anything it reads or estimates is a draft for you to check, and it can be wrong. That service is not ours, and its own terms and privacy policy apply to what you send it.

5. Acceptable use

Do not misuse, reverse engineer (except where the law allows) or resell the app.

6. Subscriptions

• Plans. Forma Core is an auto-renewing subscription, monthly or yearly. It includes both coaches. The price is shown in the app, taken from the App Store for your country, before you subscribe.
• Free trial. Forma is free for 14 days with everything switched on. The trial starts when you finish the first-run questions and asks for no payment details.
• After the trial. If the trial or a subscription ends, Forma becomes read-only. Your history, charts, export, backup and restore, "Delete all my data" and Settings keep working. Nothing you logged is deleted.
• Billing. Payment is charged to your Apple Account when you confirm the purchase. The subscription renews automatically unless it is cancelled at least 24 hours before the end of the current period, and your account is charged for renewal within 24 hours before the end of the period, at the same price.
• Cancel or manage. You cancel through Apple: in the Settings app on your iPhone, tap your name, then Subscriptions. You can also use Manage Subscription in Forma.
• Restore Purchases. In Settings > Subscription.
• Refunds. Payments and refunds are handled by Apple.

7. Warranty and liability

The app is provided "as is" without warranties. To the extent the law allows, we are not liable for indirect or consequential loss, and our total liability is limited to what you paid for the app in the last 12 months. Nothing limits liability that cannot be limited by law.

8. Changes and law

We may update these terms; continued use means you accept them. Your subscription is also covered by Apple's standard terms (Apple's Licensed Application End User License Agreement) and the App Store's terms of sale, which apply alongside these.
